This study aimed to conduct a network analysis of convergence and divergence among key stakeholders in Iran's health-oriented sports sector and identify potential coalitions and conflicts. Given the rise of non-communicable diseases and the critical role of physical activity in prevention, the development of health-oriented sports has become a national priority. This applied research employed a mixed-methods approach (qualitative-quantitative) and innovatively utilized stakeholder analysis and the MACTOR tool to examine the complex relationships among sports policymaking institutions for the first time in this field. Data were collected through semi-structured interviews with 24 experts and matrix questionnaires (actor-actor and actor-goal). Validity and reliability were ensured via qualitative strategies such as audit trail and member checking. The findings revealed that the Ministry of Health, Medical Education, and Medical Services holds the highest influence, while the Health-Oriented Sports Federation exhibits the greatest dependency. New patterns of convergence (e.g., collaboration between the Public Sports and Physical Fitness Federations) and divergence were uncovered, which had not been systematically identified before. Goal distance analysis also indicated that some key objectives, such as institutionalizing prevention through legislation, had low alignment with other goals.The primary innovation of this study lies in proposing a framework for conflict management, goal alignment, and the formation of functional coalitions among policymaking institutions. Based on this innovation, practical strategies are suggested, including the establishment of a national inter-sectoral task force, the development of a comprehensive health-oriented sports policy document, and the design of collaborative projects between ministries and federations.
